{"id":384806,"date":"2021-02-25T15:12:04","date_gmt":"2021-02-25T18:12:04","guid":{"rendered":"https:\/\/revistapesquisa.fapesp.br\/?p=384806"},"modified":"2021-02-25T16:47:05","modified_gmt":"2021-02-25T19:47:05","slug":"no-rest-on-the-anniversary","status":"publish","type":"post","link":"https:\/\/revistapesquisa.fapesp.br\/en\/no-rest-on-the-anniversary\/","title":{"rendered":"No rest on the anniversary"},"content":{"rendered":"<p>One of the largest vaccine-producing centers in Latin America, the Oswaldo Cruz Foundation (FIOCRUZ), is celebrating its 120<sup>th<\/sup> anniversary in the spotlight. Amid a seemingly endless pandemic, the institution was quick to respond to the new health care crisis, much as it has done in the past whenever necessary. \u201cWe had been preparing for the arrival of the virus in Brazil since January,\u201d says sociologist N\u00edsia Trindade Lima, president of FIOCRUZ. All FIOCRUZ units sprang into action to fight the pandemic, including the two hospitals in Rio de Janeiro, the Fernandes Figueira Institute (IFF), and the Evandro Chagas National Infectious Diseases Institute (INI). In the latter, a hospital center dedicated to COVID-19 was built. INI also leads the Solidarity Trial in Brazil\u2014a World Health Organization (WHO) initiative to accelerate results on the best therapies. \u201cFIOCRUZ has played a central historical role in addressing epidemics in Brazil. This experience is now being fully used against the novel coronavirus,\u201d says Lima.<\/p>\n<p>In September, through its Immunobiological Technology Institute (Bio-Manguinhos), FIOCRUZ and the Anglo-Swedish biopharmaceutical company AstraZeneca signed a technological order agreement, which will ensure Brazil\u2019s self-sufficiency in producing the vaccine candidate known as AZD 122, which is under development by the University of Oxford, UK. Bio-Manguinhos is set to start producing it in January 2021; 210 to 265 million doses are expected to be made throughout the year. Due to the severity of the pandemic, the normal process for the approval and planning to produce possible immunizing agents against the new coronavirus were accelerated around the world in parallel with phase 3 trials, some of which are still in progress. At the same time, FIOCRUZ research units are focusing on national projects for vaccines, therapies, and tests for rapid diagnosis of the disease.<\/p>\n<p>Public health crises are an integral part of the foundation\u2019s history, originally established as the Federal Soroterapic Institute in 1900 for combating the bubonic plague. S\u00e3o Paulo physician Oswaldo Cruz (1872\u20131917), technical director of the Soroterapic Institute, perfected and quickly began to manufacture a vaccine for the bubonic plague, developed by the French at the time\u2014and envisioned, in Manguinhos, an institution akin to the Pasteur Institute in France, where he had studied from 1897 to 1899 (<em>see page 58<\/em>).<\/p>\n<p>Today, Bio-Manguinhos annually supplies seven of the 17 vaccines included in the national vaccination calendar: yellow fever, 10-valent pneumococcal conjugate (PCV10), inactivated polio (IPV), oral polio (OPV), rotavirus (RV), measles, mumps, and rubella (MMR), and measles, mumps, rubella, and varicella\/chickenpox (MMRV). Some years, it also provides the <em>Haemophilus influenzae b<\/em> (Hib) vaccine. \u201cIn 2019, around 109 million doses were supplied to the National Immunization Program,\u201d shares administrator Maur\u00edcio Zuma, director of Bio-Manguinhos. The institute also exports the yellow fever vaccine to 75 endemic countries. In addition to vaccines, FIOCRUZ also produces reagents (diagnostic kits) and biopharmaceuticals in its immunobiological production unit, and about 30 different types of medications in the Institute of Drug Technology (Farmanguinhos). In 2020 FIOCRUZ had a budget of R$4.4 billion. However, there was an extra R$3.1 billion available through provisional measures to combat the pandemic. And, outside the Treasury budget, it received R$470 million from donations.<\/p>\n<p>In addition to production, FIOCRUZ focuses on research, teaching, assistance, dissemination, and preservation of scientific and historical heritage. The foundation is now in 11 states through 16 technoscientific units, as well as five offices. In 2017, it established the Translational Medicine Platform\u2014a partnership with the University of S\u00e3o Paulo School of Medicine in Ribeir\u00e3o Preto (FMRP-USP). One of them is a vaccine that uses antigen fragments (particles or molecules that stimulate the production of antibodies), which provoke the best immune response; the other is based on synthetic peptides (protein fragments) from parts of coronavirus proteins, which induce antibody production. Both are in the pre-trial stage, informs biologist Sotiris Missailidis, deputy director of Technological Development at Bio-Manguinhos.<\/p>\n<p>The Ren\u00e9 Rachou Institute, a branch of the foundation in Minas Gerais (FIOCRUZ Minas), is developing its own project alongside the FIOCRUZ branch at FMRP-USP: using the influenza virus to produce a recombinant vaccine, obtained through genetic engineering. &#8220;We are modifying an influenza virus, H1N1, to express the spike protein of the coronavirus, targeted by the immune system antibodies,\u201d summarizes biochemist Ricardo Gazzinelli, coordinator of the National Institute of Science and Technology in Vaccines (INCTV) and head of the project. The result could be a bivalent vaccine\u2014currently being tested on mice\u2014that can protect against both diseases. Alongside this project, there are three other vaccine studies underway coordinated by Gazzinelli\u2014against Chagas disease, leishmaniasis, and malaria.<\/p>\n<p>At the Gon\u00e7alo Moniz Institute (FIOCRUZ Bahia), the main attraction is the research on stem cells, used in cell therapy for degenerative and traumatic diseases. The studies started in 2002, with physician Ricardo Ribeiro dos Santos and immunologist Milena Soares\u2014now head of the Laboratory of Tissue Engineering and Immunopharmacology (LETI). One of the lines of research investigates the pathogenesis mechanism of the Zika virus using pluripotent stem cells, with the goal of discovering new treatments. Another, more recent research line, investigates the application of cell therapy as an alternative treatment for lung disease caused by the inflammatory process of severe COVID-19 cases.<\/p>\n<p>Sharing knowledge for the benefit of public health has been a mission of the institution since Oswaldo Cruz&#8217;s time. The doctor and researcher was passionate about making the smallpox vaccine mandatory\u2014which resulted in a strong public response known as the Vaccine Revolt, in 1904. At that time, the scientist wrote informative leaflets and educational articles in the press, titled &#8220;Advice to the People,\u201d for his campaigns against yellow fever, smallpox, and bubonic plague.<\/p>\n<p>According to historian Jaime Larry Benchimol, a researcher at the Oswaldo Cruz House (COC)\u2014a FIOCRUZ branch dedicated to the history of the institution\u2014research, teaching, and production were \u201cthe three cornerstones\u201d of the institution since its inception. The S\u00e9rgio Arouca National School of Public Health (ENSP), also part of the foundation, offers master\u2019s and PhD programs, as well as specialization and professional qualification courses (refresher courses, advanced training, and continuing education), both in person and remotely. But it is not the only branch of the institution dedicated to teaching. &#8220;All 16 units have educational activities, even those focused on production, such as Farmanguinhos and Bio-Manguinhos,\u201d highlights the vice president.<\/p>\n<p>One of the educational branches focuses on scientific dissemination, carried out through initiatives such as the master\u2019s Program in Dissemination of Science, Technology, and Health, offered by the COC, and the production of magazines and books, many of which are available at no cost through the SciELO electronic library and through Arca, the institutional repository of FIOCRUZ. The dictatorship of Get\u00falio Vargas (1937\u20131945) was a traumatic break in this relationship, and the first major crisis at the institute.<\/p>\n<p>According to the historian, until the Estado Novo (New State), the IOC had the power to generate and manage its own income from the sale of vaccines and services. \u201cWe had money mainly from the sale of the vaccine to prevent symptomatic carbuncle in Brazilian cattle. This money paid the salaries of many of the researchers, as well as part of the construction of the institute\u2019s hospital on campus, scientific expeditions, and other activities,\u201d he explains. Having financial autonomy meant the institute could survive regardless of political interests. The Estado Novo revoked this prerogative and the institution largely lost its autonomy.<\/p>\n<p>\u201cRural endemic diseases, as well as infectious or chronic conditions, started to be studied at universities, at the institute established in Bel\u00e9m [Par\u00e1] by Carlos Chagas&#8217;s [1879\u20131934] firstborn son, Evandro Chagas [1905\u20131940], and at the institutes in Recife, Bahia, and Belo Horizonte, part of the National Institute of Rural Endemic Diseases, founded in the 1950s,\u201d reports Benchimol. In 1970, some of these Ministry of Health institutions were consolidated under the name Oswaldo Cruz Institute Foundation (FIOCRUZ), then simply Oswaldo Cruz Foundation. That same year, the institution experienced another crisis: the so-called Manguinhos Massacre. Ten of the foundation scientists, recognized for their scientific contributions, were forcibly retired by the military regime (1964\u20131985), leading to the interruption of research and the closure of their laboratories. These were: Augusto Cid de Mello P\u00e9riss\u00e9, Domingos Arthur Machado Filho, Fernando Braga Ubatuba, Haity Moussatch\u00e9, Herman Lent, Hugo de Souza Lopes, Masao Goto, Moacyr Vaz de Andrade, Sebasti\u00e3o Jos\u00e9 de Oliveira, and Tito Arcoverde de Albuquerque Cavalcanti.<\/p>\n<p>The institution would again play an important role during the government of Ernesto Geisel (1974\u20131979), in the context of another health crisis: a meningitis epidemic that forced the government to import 80 million doses of vaccine from the M\u00e9rieux Institute in France, for a mass vaccination campaign in 1974. The doctors linked to ENSP would have a decisive role in the movement that resulted in the establishment of SUS. S\u00e3o Paulo health surveillance physician S\u00e9rgio Arouca (1941\u20132003), president of the foundation from 1985 to 1989, was one of the leaders of this movement. In 1986, in the opening speech of the 8<sup>th<\/sup> National Health Conference, where the foundations of SUS were defined, Arouca defended an integrated view of health care, related to a social well-being that was only possible in a democracy: \u201cHealth is not simply the absence of disease; it is much more than that. It means mental, social, and political well-being. (&#8230;) It is the result of a just economic and social development.\u201d In 1988, the Constitution would confirm that in its Article 196: \u201cHealth care is a right for all and a duty of the state.\u201d<\/p>\n<p>In 2014, FIOCRUZ established the Strategic Studies Center (CEE), linked to the institution&#8217;s presidency, to encourage and guide the development of health care policies. The president of the foundation highlights its prospective studies to identify and anticipate scenarios that may affect the development of the institution and the country.<\/p>\n<p>After 120 years, FIOCRUZ continues to focus on the present, but always looking to the future. In 2019, it began a research project in Antarctica, FioAntar, which became part of the Brazilian Antarctic Program. The goal is to investigate microorganisms that cause diseases or have biotechnological potential on the frozen continent. Scientists from eight foundation laboratories are participating in the project.<\/p>\n<div class=\"box\"><strong>Autonomy: The goal <\/strong><\/p>\n<p>An industrial complex may reduce our dependence on imported inputs \u201cWe must be less dependent on other countries,\u201d argues immunologist Milena Soares, from FIOCRUZ Bahia, expressing a concern that has motivated FIOCRUZ to generate scientific and technological knowledge focused on public health. Pharmacologist Rubens do Monte, from FIOCRUZ Minas, believes Brazil\u2019s dependence on imports has been exposed by the current pandemic: \u201cDiagnostic supplies for COVID-19 are all imported,\u201d he explains. To reduce this dependency, he is working on a project to produce testing swabs through 3D printing. If the materials are produced on a large scale, they could be an alternative supply for public health units across the country.<\/p>\n<p>\u201cThe pandemic has demonstrated the importance of SUS [Brazil\u2019s Unified Health System], but it has also exposed its weaknesses, such as technological dependence on imported inputs\u2014up to 90% for drugs and PPE [Personal Protective Equipment] and about 80% for ventilators and other equipment,\u201d says N\u00edsia Trindade Lima, president of FIOCRUZ. &#8220;We defend the idea of an industrial complex as a solution to this problem.&#8221; To produce vaccines in Brazil, the Industrial Biotechnology in Health Complex (CIBS)\u2014currently under construction on the west side of the city of Rio de Janeiro\u2014will be the largest vaccine factory in Latin America, with an estimated production capacity of 120 million vials of immunizing agents and biopharmaceuticals per year.
